Report Close Contact or Positive COVID-19 Test

Report exposure and positive test results through the secure portal:

  • Go to
  • Choose “send a secure message” to the “nurse”
  • Report what date your symptoms started
  • Report what date you tested positive
  • Attach a picture of your test report or your home test.
