New federal guidance issued this week has extended the COVID-19 vaccination deadline for UAH faculty, staff, and student employees to Jan. 4, 2022.

Employees, including all student employees, requesting a medical disability or religious exemption are strongly encouraged to submit their completed request by Nov. 19, 2021.

As a reminder, all UAH employees, including student employees, must complete a COVID-19 Vaccination Status Release survey regarding permission to share vaccination status with UAH Human Resources or Student Affairs. We encourage all employees to complete the survey now, even if you have not yet been vaccinated.

UAH currently has free Pfizer, Moderna and Johnson & Johnson COVID-19 vaccines available. Appointments can be made through the UAH Health Portal or by calling 256.824.2400.

Employees who receive COVID-19 vaccines from an off-campus provider must also upload their vaccine card to the UAH Health Portal or bring a copy of the card to the Faculty Staff Clinic or Student Health Center. If you have already uploaded/submitted your vaccine card, you do not need to submit it again. Detailed instructions on how to upload a vaccine card to the secure portal can be found here.

UAH will be closed Dec. 24 – Jan. 3, 2022, and UAH strongly encourages all employees to take action before the break. However, employees can still upload vaccine cards to the UAH Health Portal, complete the COVID-19 Vaccination Status Release and submit exemption requests to Human Resources at hr@uah.edu during this time.
