Have you completed all the wellness tiers to receive the $75 incentive? Remember you must complete both requirements in Tier 1 before you can earn additional points for Tier 2 and Tier 3. See below for the breakdown of the Tier levels. All regular full-time faculty and staff are eligible to participate.
Tier 1: Complete the following activities and you will receive a $25 wellness incentive.
- Health Assessment: Available through the online wellness portal (25 points)
- Biometric Screening: Complete at the Faculty and Staff Clinic or your primary physician (25 points). The UAH screening form must be turned in to the Faculty and Staff clinic to receive points.
Tier 2: Complete requirements for Tier 1 and earn 100 additional points (150 total points) to receive a $50 wellness incentive.
Tier 3: Complete requirements for Tier 2 and earn 200 additional points (250 total points) to receive a $75 wellness incentive.
