Important Notice about our Google Forms

    You must use your UAH email address and password to sign into the Google Forms below. If your browser is already signed into your personal email account, Google Forms will require you to request access to the form. We do not allow access to anyone outside of the UAH email domain. Opening a private or incognito tab to access the form is a simple workaround.

Students have until the end of the first two weeks of the Fall semester (September 2, 2026) to request any changes to their dining options. All change requests are processed manually.
