April 28, 2020 Faculty and Students, The IRB committee has updated the IRB forms 1 through 9 and the consent form. These forms are now uploaded on the IRB website. Forms 2, 3, 6, 7, and 8 are now in a fillable format. Please use the updated forms when submitting an IRB application. We have also added a new form. Form 10: The Youth Assent Form and Youth/Guardian Consent Form: This consent form is to be used when you are including children in your study. This form provides the child the opportunity to give assent and also provides the parent with information so they can give their consent for their child to participate in the study. Page 1 includes instructions and the Youth Assent form to be completed by the child and should be written in a language that is developmentally appropriate for the child so they can understand and express their willingness or unwillingness to assent to participate.
