The IRB has added an additional form addressing the use of Translators. Form 13: Translator’s Declaration form will need to be included into the IRB Application when the research team includes a specific individual that will be responsible for translating documents from English to other languages. The translator will need to fill out Form13: Translator’s Declaration form and select the appropriate role as the translator. This form will be uploaded within the next few days.

CITI training is now requiring Chairs or PIs to access the link below so they may initiate a process for non-affiliated members to gain access to CITI Training.  If an individual is not affiliated with an institution that has IRB  and is engaged in the research (obtaining consent, collecting data, have access to identifiable data) then the non-affiliated individual will need to have access to CITI training. The PI or Chair will need to access the link below that will provide the non-affiliated member with a UAH SSO so they will get access to CITI training. 

When completing the form the Chair or PI is designated as the Sponsor and the non-affiliated member is designated as the contractor.

The non-affiliated individual will also need to complete and submit the Individual Investigator Agreement form (located on the IRB website under forms) and submit this form along with the CITI training certificates with the IRB application submission.
