January 21, 2022 The IRB committee has approved a new form titled: Form 12: Guidance: Model Telephone Consent. This form is now uploaded on the IRB website under forms. https://www.uah.edu/irb/forms If you will be obtaining consent via telephone you will need to include this form in your IRB application. Form 12 outlines the information required when obtaining consent for prospective participants via the telephone. The form includes a sample statement that provides the required information needed to gain consent over the telephone. Use the sample statement to frame your telephone script. Just a reminder the Consent form has been updated and now includes a Key Information section at the beginning of the consent form. Be sure to respond to each bullet point in this section. This is required by federal law and will need to include the specific information pertaining to your study.
