Below is a list of forms for Instructional Testing Services: Campus Services: Offering time accommodation, make-up, and deferred final examination services for UAH faculty and students. Test Administration Instructions states proctoring guidelines and should accompany all exams submitted to Instructional Testing Services. Testing Scanning Form states instructions for processing/grading class scantron packets. Online Learning Services: Offering UAH Online Learning campus proctoring services and assist UAH faculty with facilitating remote student examinations. Online Learning Proctor Instructions states proctoring guidelines for all UAH Online Learning exams serviced by Instructional Testing Services. Remote Proctor Agreement states the agreement that must be made to become a UAH approved remote proctor. Late Arrival Policy: Exams start at 9:00 am and 1:00 pm Monday- Friday. To reduce distractions during exam blocks, students are required to arrive 15 minutes prior to their appointment Students who arrive after exams have started are considered late. In the event a student is unable to arrive on time, he/she must contact the Instructor for rescheduling options. A cumulative of 3 tardies and/or missed appointments during the semester will result in loss of testing privileges in Testing Services for the remainder of the semester. Should this occur, students will be referred back to their Instructor.
