UAH offers the long term disability (LTD) insurance to all regular full-time employees. Long Term Disability (or Salary Continuance) insurance coverage is applicable when one is completely unable due to sickness, bodily injury, or pregnancy to perform the material and substantial duties of his/her normal occupation and unable to perform any other occupation for which one is reasonably qualified by education, training, or experience. One must be under the regular care of a physician so that a reasonable program of care and treatment can be maintained. The program is designed for individuals who have a long term illness usually lasting more than 90 days. Who is Eligible? All regular, full-time employees are covered by the LTD program. What are the benefits? There is a 90 day benefit waiting period. After the benefit waiting period if LTD is approved the monthly benefit is 66 2/3% for the first 90 days, then 60% of your insured predisability earnings reduced by deductible income. The plan monthly maximum benefit is $10,000 per month. The plan minimum benefit is $ 100 per month. What does it cost? The premiums for this insurance are paid entirely by the University. No employee cost. Special Features Survivor Income Benefit - A survivor Benefit will be paid to surviving dependent(s) for three months after your death if you had been disabled for at least nine months. Rehabilitation Services - Rehabilitation services prepare you to work to the fullest extent of your ability. The services may include but are not limited to vocational testing, job preparation, career counseling, retraining, and work place modification. Conversion Privilege - If you terminate employment, there is a 31-day-conversion period during which you could purchase the LTD insurance without providing proof of good health. Conversion is not allowed when termination is due to retirement. Please refer to the terms relating to conversion privileges in your LTD booklet.
