Thank you to all vaccinated faculty and staff who took a few moments to upload a photo of your COVID-19 vaccination card to the secure UAH Health Portal.

The UA System has updated their COVID-19 Dashboard: The UA System's vaccination data includes information from faculty/staff health centers and voluntary self-reports. This data reflects individuals who have received at least one dose of the COVID-19 vaccine. 

If you have been vaccinated off campus and have yet to upload your documentation, please do so. My hope is that UAH’s vaccination rate will continue to climb.

The confidentiality of your personal information will be protected and only our healthcare professionals bound by HIPAA will have access to your secure information.  While it is not required, we would certainly appreciate your willingness to upload your proof of vaccination to your protected healthcare record.

To do so, simply take a picture of your vaccination card and upload it to our secure portal at: UAH Health Portal. After entering your Charger ID and password, choose Upload Immunization. The portal can also be accessed from either Student Health or Faculty & Staff Clinic websites. If you are unable to upload a picture, you can bring a copy of your card to the healthcare professionals in the UAH Faculty & Staff Clinic. Thank you, in advance, for your participation.

If you are not yet vaccinated, please consider getting vaccinated as soon as possible. Data continue to show the vaccine provides strong protection against infection – especially serious complications – compared to those who are unvaccinated. Appointments can be made through the UAH Health Portal.
