ACTION IS REQUIRED if you want to: Participate in a Health Care Flexible Spending Account (HCFSA) Participate in a Dependent Care Flexible Spending Account (for childcare expenses- DCFSA) Make your own contributions in your Health Savings Account (HSA) Make changes to your existing insurance elections for medical, dental, or vision insurance NO ACTION IS REQUIRED if you do not want to make changes to health, dental, vision, life, or disability coverage.
