The clinical experiences of nursing students require a health screening program. The following steps are required as part of admission to and enrollment in the graduate program:

The College of Nursing contracts with an outside agency to maintain access to your documentation requirements. Please be prepared to upload all documents below during orientation:

  • MMR Two Dose Vaccines, or Mumps Titer, Measles Titer, and Rubella Titer.
    If non-immune titer, Repeat Two Dose Vaccine
  • Hepatitis B Three Dose Vaccines, or Hepatitis Titer
    If non-immune titer, Repeat Three Dose Vaccine
  • Varicella Two Dose Vaccines, or Varicella Titer
    If non-immune titer, Repeat Two Dose Vaccine
  • Tetanus Diphtheria, Pertussis (Tdap)
    Tdap or TD booster required every 10 years after initial TDap. May never be more than 10 years old during matriculation.
  • Influenza Vaccine
    Required Seasonally. Must be submitted by 10/31 of year of admission and maintained through matriculation.
  • Tuberculosis Two-Step Negative Skin Tests (PPD/Mantoux) or QuantiFERON TB Gold
    If you have had initial testing and annual screening through your employer, documentation of annual screening and a single step will be sufficient.
  • Chest X-Ray if previous positive
    If TB Skin test is positive, a chest x-ray must have been completed with the last 5 years (annual symptom evaluation must be completed and uploaded).
  • Completion of three modules in "Culturally Competent Nursing Care: A Cornerstone of Caring" found at https://ccnm.thinkculturalhealth.hhs.gov/default.asp
  • Proof of Health Insurance
  • Physical/Health Examination Form
  • CPR Certification by the American Heart Association
  • Proof of Liability Insurance
  • For Adult-Gerontology Acute Care Nurse Practitioners (AGACNP):
    • ACLS Certification by the American Heart Association
  • OSHA Certification
  • HIPAA Certification
  • Signed consent forms for:
    • Substance Abuse Policy
    • Drug/Alcohol Testing and Sentry MD
  • Unencumbered Nursing License
