ACTION IS REQUIRED IF YOU WISH TO:
- Participate in a Health Care Flexible Spending Account (HCFSA)
- Participate in a Dependent Care Flexible Spending Account (DCFSA) for childcare expenses
- Participate in a Health Savings Account (HSA)
- Make changes to your existing insurance elections for medical, dental, or vision insurance
NO ACTION IS REQUIRED if you do not want to make changes to health, dental or vision coverage. You will keep your current benefits and these coverage(s) will rollover as-is effective January 1, 2026.
