Employee Eligibility

Medical, dental, and vision insurance are available to all full-time regular employees and their eligible dependents. Temporary full-time employees also have the option to enroll, but part-time employees who work less than 75% time are ineligible to participate in this program. New employees who wish to participate in the University’s health, dental, vision insurance and Flexible Spending Accounts must apply for coverage within 31 days of employment. Coverage for a new employee becomes effective on the first of the month following the date of hire.

Individuals wishing to add or delete dependent coverage due to a change in family status (marriage, divorce, birth of a child, etc.) must notify a benefits representative in the Office of Benefits & Employee Services within 31 days of the status change. Otherwise, applications to add or delete coverage will be accepted only during the annual open enrollment period with coverage to be effective January 1 of each year.

Dependent Eligibility

General definitions of eligible dependents are given below.

  1. Spouse – Lawful spouse
  2. Dependent Child – Child (up to age 26) of the employee or employee’s lawful spouse to include the following:
    • Biological child
    • Step child
    • Legally adopted child (including a child placed for adoption)
    • Child for whom the employee has a final custody order or final disposition of legal custody
    • Incapacitated child (over age 26, unmarried, and permanently incapacitated before age 26; child is not able to support him/herself and dependent on UAH employee for his/her support due to this incapacity).

Required Documentation

Spouse: A copy of your marriage certificate.

Dependent Children: A copy of the child's birth certificate or appropriate court order/adoption/divorce decree naming you or your spouse as the legal guardian.
