Schedule your appointment though the UAH Health Portal:

Click "Schedule an Appointment"

Select "Free FLU vaccines 1/28/2021"

Select "No, I would like an appointment another day (1/28/2021)"

Follow instructions to register for the appointment.

 

On the day of your appointment you will receive a text and/or email with a link to fill out the required flu vaccine consent form.  This form MUST be completed before your appointment.
