Only with your help can we collect the data needed to continue making informed decisions about our fall semester campus activities. I encourage all vaccinated faculty and staff who were vaccinated off-campus to take a few moments to upload a photo of your COVID-19 vaccination card to the secure portal.
My hope is that UAH will have a high vaccination rate to share publicly; however, we need your help collecting the pertinent information.
The confidentiality of your personal information will be protected and only our healthcare professionals bound by HIPAA will have access to your secure information. While it is not required, we would certainly appreciate your willingness to upload your proof of vaccination to your protected healthcare record.
To do so, simply take a picture of your vaccination card and upload it to our secure portal at: UAH Health Portal. After entering your Charger ID and password, choose Upload Immunization. The portal can also be accessed from either Student Health or Faculty & Staff Clinic websites. If you are unable to upload a picture, you can bring a copy of your card to the healthcare professionals in the UAH Faculty & Staff Clinic. Thank you, in advance, for your participation.
If you are not yet vaccinated, please consider getting vaccinated as soon as possible. Data continue to show the vaccine provides strong protection against infection – especially serious complications – compared to those who are unvaccinated. Appointments can be made through the UAH Health Portal.
