The student and the instructor need to work together to determine when exams will be administered for each class. Some instructors prefer tests to be at the same time as the in-class test. Student Responsibilities Notify the Testing Services Center (in person, via email or by phone) that they need to schedule a test. TSC only administers tests M-F between 8:30-4:30. Exams must be completed by 4:30pm. This notification should be no later than 1 WEEK before the test is to be administered. DSS is not responsible for proctoring tests with insufficient notice. In these cases the student will be responsible for taking his/her test with the class and the instructor will be responsible for providing the student with the accommodations. Schedule a time/date. Complete the first section of the Accommodated Exam Proctoring Form and send it to faculty. Come to TSC at the appointed time. Students who arrive late for their test may take the test, but WILL NOT be given time beyond that scheduled for the test (e.g. a test scheduled to end at 2:00 p.m. will end on time regardless of the start time if the student was late). Bring ONLY standard testing supplies. Books, bags, cell phones, papers, etc. may not be taken into the testing room. TSC is not responsible for belongings left in the office during or after the test. TSC has a series of small lockers that students can use during testing to store belongings. Abide by university conduct expectations. Instructor Responsibilities Complete the Accommodated Exam Proctoring Form for each test to be administered. Student will complete the first section of the form and give it to faculty to complete. We will no longer accept tests with no cover sheet (Accommodated Exam Proctoring Form). It is unclear what the professor’s desires are without the form. Deliver (in person, by designated person, or via email) the test along with the Accommodated Exam Proctoring Form to TSC 48 hours prior to the test time. TSC is not responsible for proctoring tests with insufficient notice. In these cases the student will be responsible for taking his/her test with the class and the instructor will be responsible for providing the student with the accommodations. Deliver any additional testing materials (charts, scratch paper, etc as part of the test.) TSC will not supply the student with any supplies, including scratch paper, pens, pencils, calculator, etc. Pick up the test after it has been administered (either in person, by designated individual, via scan-and-email, or fax).
