Exam Administration Procedures Spring 2009
Exam Administration Form
Student Responsibilities:
- The student and the instructor need to work together to determine when exams will be administered for each class. Some instructors prefer tests to be at the same time as the in-class test.
- The STUDENT is responsible to:
- Notify DSS (in person, via email or by phone) that they need to schedule a test. DSS only administers tests M-F between 9-4.
- This notification should be no later than 1 WEEK before the test is to be administered. DSS is not responsible for proctoring tests with insufficient notice. In these cases the student will be responsible for taking his/her test with the class and the instructor will be responsible for providing the student with the accommodations.
- Schedule a time/date.
- Come to the DSS office at the appointed time
- Students who arrive late for their test may take the test, but WILL NOT be given time beyond that scheduled for the test (e.g. a test scheduled to end at 2:00 p.m. will end on time regardless of the start time if the student was late).
- Bring ONLY standard testing supplies.
- Books, bags, cell phones, papers, etc. may not be taken into the testing room. DSS is not responsible for belongings left in the office during or after the test.
- -
- Remain in the testing room during the duration of the exam. The test will end when the student leaves the room.
Instructor Responsibilities:
- Complete the Exam Proctoring Checklist for each test to be administered. We will no longer accept tests with no cover sheet (exam proctor checklist). It is unclear what the professors desires are without the form, furthermore we need to note a preferred delivery format
- Deliver (in person, by designated person) the test along with the Exam proctoring checklist to DSS 48 hours prior to the test time.
- DSS is not responsible for proctoring tests with insufficient notice. In these cases the student will be responsible for taking his/her test with the class and the instructor will be responsible for providing the student with the accommodations.
- Deliver any additional testing materials (charts, scratch paper, etc as part of the test.) WE WILL NOT PROVIDE THE STUDENT WITH ANY SUPPLIES including scratch paper, pens, pencils, calculator, etc.
- Pick up the test after it has been administered (either personally or by designated individual) or choose the Friday delivery or fax options.
