If, as a requirement of the NASA Contract under
which you are working, you will require access to the Marshall
Space Flight Center (MSFC) facilities on
a recurring and continuing basis (at least three times per week), you may be eligible to obtain a MSFC
Contractor Badge. The Marshall Space Flight Center Contractor
Badge/Decal Application form (MSFC Form 1739) may be obtained from our office. There are two versions of this form. The
prime
contractor version of MSFC Form 1739 is for use when UAH is the prime contractor on the
NASA contract. The subcontractor version of MSFC Form 1739 is for use when UAH is
a subcontractor on the NASA contract. We provide these versions
online merely so that you know what the form looks like. The
online versions may not be used to complete this badge application
process.
You will be able to obtain the correct NASA Contract Number under
which you will be working and determine whether UAH is the prime
contractor or a subcontractor by contacting the UAH
Office of Research Administration.
If you are a foreign national, you must complete
an approval process for access to MSFC prior to submitting the request for a Contractor
Badge.
Follow the instructions below for
completing the MSFC Form 1739 as either a prime contractor or a
subcontractor. You may use the navigation tool provided to
access information regarding a particular portion of the form.

This form is to be typed or printed using
black ink only except in the "Residence Address" block
which is to be completed in pencil only.
On the front of the form, leave blank the block marked
"Badge Number."

- On the first full line of the front of the
form, indicate your LAST name in ALL CAPITAL letters in the
block marked "Last." Indicate your first name
in the block marked "First" using initial capital
letters only. Indicate your middle initial in the block
marked "Middle Initial." Indicate your Social
Security Number in the block marked "Social Security
Number."
- On the next full line, indicate your name (as you would
like it to appear on your badge) on the line in that block.
- Indicate your date of birth in the block marked "Date
of Birth." Use a MM/DD/YY format.
- Indicate your place of birth in the block marked
"Place of Birth." If born in the U.S., indicate
city and state of birth as well as USA. If born outside the
U.S., indicate city and country of birth.
- Indicate your citizenship in the block marked
"Citizenship."
- Mark the correct square indicating Male or Female for your
gender in the square marked "Sex."
- In the block marked "Height," indicate your height in
INCHES.
- In the block marked "Weight," indicate your weight in
POUNDS.
- Indicate your current hair color and eye color in the
blocks marked "Color Hair" and "Color Eyes",
respectively. Spell out the word completely. Do NOT
use abbreviations.
- Using PENCIL ONLY, print your current, local home address in the
block marked "Residence Address."
- Indicate your job title, work telephone number, and home
telephone number in the blocks marked "Job Title or
Position," "Work Phone Number," and "Home
Phone Number," respectively.

- If you are working on a prime contract,
in the portion of the form marked "TO BE COMPLETED BY
CONTRACTOR SECURITY OFFICE," the company name and address
should already be entered as follows:

portion of the form marked "TO BE COMPLETED BY CONTRACTOR

supply the Contract Number under which you will be working.

Do NOT complete the block marked
"Expiration Date." This expiration date will be
supplied by the UAH Office of Research Administration
personnel.

Leave Blank the signature blocks for the Prime
Contract Security Officer and the Subcontract Security Officer.

You will only complete the back
side of the form if you require a NASA parking decal. Provide
the information requested on
the back of the MSFC Form 1739 regarding the Year, Make, Model,
Color, and License Plate of each vehicle you will be using to access
the facility. You must provide ALL information requested for
ALL vehicles listed.

Sign your name in the block marked "Signature
of Contractor" and indicate the current date in the block
marked "Date." Note: If you are not applying
for a NASA decal, do not sign this side of the card.

When you have completed the MSFC Form 1739 form to
this point, present it to the UAH Office of Research Administration
for verification of Contract Number and completion of the expiration
date. The staff member in the Office of Research Administration that
completes the expiration date will initial that block to indicate
his or her review/verification.

After the UAH Office of Research Administration has
reviewed the form and completed the expiration date, bring the MSFC
Form 1739 to our office for further processing.

After we have approved, signed,
and returned the MSFC Form 1739 to you, you will then obtain the
approval of the NASA representative authorized to sign for the
contract under which you are working. Be certain the NASA
employee is authorized to sign badge cards on this particular
contract. The UAH
Office of Research Administration will assist you in determining
which NASA employees are authorized to sign for your contract.
This NASA representative
will complete the bottom third of the form and will complete the
block marked "Date Badge Should Expire." This may or
may not be the same date as the expiration date of the
contract. This block is completed at the discretion of the
NASA representative.

After obtaining the NASA representative's signature,
you will take the form to the NASA Badging Office in Bldg. 4312,
where your badge will be made. Take with you proof of
ownership and proof of insurance for all vehicles for which you
intend to obtain a NASA parking decal. A reprint of a Redstone

Upon receiving your badge, contact
us to provide your Badge Number and expiration date so that we
may assist you with renewal of your badge when necessary. After you have obtained this badge, if you are also applying for
RSIC privileges, you will need to come by our office in person to
bring us your RSIC Patron
Registration Form with the NASA Badge No. and expiration date completed as indicated on that card.
We cannot process your RSIC Patron Form without this information.
If you know you will need RSIC privileges based on this NASA badge,
you may complete the MSFC Contractor Badge/Decal Application Form
and the RSIC Patron Registration Form simultaneously, but the NASA
Badge must be obtained before the RSIC Patron Registration Form is
turned in to RSIC because that form must contain your Badge No. and
expiration date.
