| The Electronic Personnel Security Questionnaire (EPSQ)
is essentially an electronic version of the Standard Form 86 which was
used in the past as a personnel security questionnaire. You may download
the EPSQ Subject Version to your computer from the DSS web
site. This software is subject to U.S. Export Control Laws and
you will be required to certify your agreement to abide by these
laws. If you prefer not to download the software to your
computer, you may use use a computer on which the software is loaded
in our office to create your EPSQ subject data file.

After you have completed and validated your subject data file, you may bring it to our office on a floppy disk or you may email it to the Assistant Facility Security Officer. Remember that the password you used to create the EPSQ subject data file must be provided to the Assistant Facility Security Officer regardless of the method of transmission involved. In addition, you must come by our office and sign a paper copy of the EPSQ as proof that you are in agreement with what is being submitted, and you must meet all other requirements as specified in the NISPOM. The EPSQ is a detailed examination of your past history, both personal and professional. For a SECRET clearance this examination covers the last five years. For a TOP SECRET clearance this examination covers the last ten years. Useful Worksheet | Important Tips on EPSQ | What Happens Next We recommend that you complete the SF 86 Worksheet prior to attempting to complete the EPSQ itself. This worksheet will aide you in gathering together all the information you will need to complete the EPSQ before you begin that online process. You may print this worksheet and complete it by hand. Completing the worksheet first will save you time and frustration in the long run. When you have completed the worksheet and assembled the required information, you may begin the process of completing the EPSQ. When you begin the EPSQ file creation process, you will be asked to create a password for the EPSQ file you will be creating. Remember this password because your file cannot be accessed without it and this password must be provided to the Assistant Facility Security Officer when you submit your EPSQ subject data file. If you so desire, you may create a second password under the EPSQ Utilities Menu to protect Modules 17 through 42 from view by your security officer. These modules constitute what we refer to as the "Privacy Section" of the EPSQ. This second password does not need to be provided to the Assistant Facility Security Officer. When asked to choose the type of form to be completed, choose "Request for Security Clearance [SF86]." If you are requesting a clearance at the SECRET level, choose "National Agency Check [NAC or NACLC]" when asked which type of investigation you are requesting. If you will require a clearance at the TOP SECRET level, choose "Single Scope Background Investigation [SSBI]". Be careful entering your Social Security Number because it cannot be changed or corrected once it is entered. Select "No. 2 - Contractor Clearance Required" for Organization Type when asked. Complete, validate, and submit the EPSQ subject data file according to the online instructions provided with the EPSQ software. Contact the Assistant Facility Security Officer if you have any questions. The EPSQ file and other necessary paperwork will be forwarded by the Assistant Facility Security Officer to the Operations Center - Columbus (OCC) for processing when it has been verified that the file properly validates and when you have completed all other requirements including a personal data sheet, proof of citizenship, justification form, fingerprint cards, and the authorization for a local records check. When the Letter of Consent (LOC) granting your final clearance or an interim clearance is received in our office, you will be contacted to schedule the required briefing for your clearance. If you have completed this EPSQ as part of a periodic reinvestigation, you will be informed when the updated LOC is received by our office. Home |
