| If, as a requirement of the U.S. Army
contract under
which you are working, you will require access to Redstone
Arsenal and the Aviation and Missile Command (AMCOM) facilities on
a recurring and continuing basis, you may be eligible to obtain a
Personnel Identification Badge. The Personnel Identification
Badge Request Form (SAM Form 1116) may be obtained from our office.
We provide a copy of this form online merely so that you know what the form looks like. The
online version may not be used to complete this badge application
process. Take proof of ownership and proof of
insurance for the vehicle with you when you go to the AMCOM Badging
Office (Bldg. 3423, Redstone Arsenal) to have your badge made. A reprint of a

- In Block 2 indicate your LAST
name in ALL CAPITALS followed by your first and middle names.
- In Block 3 indicate your
electronic mail address.
- In Block 4 indicate the
acronym "UAH" followed by the name of the department or
center with which you are associated.

office/work telephone number.
- In Block 6 indicate the
acronym "UAH" followed by the Army Contract Number under
which you are working.
- In Block 7 indicate the

current, local home address. Please note this is your LOCAL
address and may or may not be your permanent address.

- In Block 11 indicate your job
title as applies to the U.S. Army contract under which you are
requesting access.
- In Block 12 either check the block indicating
U.S. citizenship if you are a U.S. citizen, or check the block
indicating "Other" if you are a foreign national and
indicate your country of citizenship if other than U.S. If
you are NOT a U.S. citizen, there is an approval process that must
be completed prior to submission of this badge form. Contact

- In Block 13 indicate your
race. The categories from which you are to choose include
"White," "Asian," "Indian," and
"Black." Spell out the word completely. Do
not use abbreviations.
- In Block 14 indicate your
hair color. Spell out the color completely. Do not use

color. Spell out the color completely. Do not use
abbreviations.
- In Block 16 indicate your
gender. Spell out "male" or "female"
completely. Do not use the abbreviations "M" or
"F."

height in INCHES.
- In Block 18 indicate your
weight in POUNDS.
- In Block 19 indicate your
date of birth. Use a MM/DD/YY format.
- In Block 20 indicate your
place of birth. Indicate the city and state if born in the
U.S. or the city and country if born outside the U.S.

- Do not complete Blocks 21 through
23. These will be completed by the AMCOM badging office
personnel.
- In Block 24 if you have served in the U.S.
Armed Forces or Civil Service, check the block indicating
"VCM Required." If you have no prior service in
the U.S. Armed Forces or Civil Service, check the block indicating
"VCM Not Required."
- Do not complete Blocks 25 through
30. These will be completed by the AMCOM badging office
personnel.
- After completing the form as indicated
above, bring the form to our office for further processing.
We will ensure that a Visit Authorization Request (VAR) is sent to
the appropriate office on Redstone Arsenal. We will also
review the form and initial Block 6.

- The form will then be returned to you so that
you may obtain a signature from the individual designated by your
Contracting Officer/Technical Representative (COTR) indicating
their approval of your access.
- After obtaining the appropriate
signature from the individual designated by your COTR, you will
take the form to the AMCOM Badging Office (Bldg, 3423) to have
your badge made.
- Upon receiving your badge, contact
us to provide to us your Badge Number and expiration date so that we
may assist you with renewal of your badge when necessary.
After you have obtained this badge, if you are also applying for
RSIC privileges, you will need to come by our office in person to
bring us your RSIC Patron
Registration Form with the Army
Badge No. and expiration date completed as indicated on that form.
We cannot process your RSIC Patron Form without this information.
If you know you will need RSIC privileges based on this Army
badge, you may complete the badge request form and the RSIC Patron
Registration Form simultaneously.
