Network Forms

To connect a UAH departmental computer to the Campus Wired Network, complete the online Campus Network Access Request Form. Alternatively, you may print, fill out, and return the .pdf version to the OIT Help Desk.

Use the following forms to request installation and activation of a new network drop for a lab or faculty/staff office. 

NOTE: A completed budget transfer request must be submitted with this form before the installation can be scheduled.

To request a new domain name for a server, web page, or service, contact the OIT Help Desk.

UAH students, faculty and staff will receive an account automatically. Click here to review the policies regarding email accounts. If you have a special request, please use one of the following forms:

• Email Alias Change Request     No form required! Use the OIT User Services Portal to make your request.
