Health Requirements - Undergraduate

The following health requirements must be submitted prior to beginning nursing classes. An orientation is hosted to provide more information and any specific forms but you are encouraged to begin collecting the following data, certifications, immunizations and/or titers (Childhood immunization records are accepted).

On campus health services are provided by the Student Health Center.

MMR - Measles, Mumps, Rubeola Measles

• Immunization with two (2) doses of vaccine required or titer results demonstrating immunity is required.
• Date and copy of immunization proof or titer results are required.
• Proof of titer must include 1) healthcare provider signature, 2) date of signature, 3) license # or clinic stamp information and 4) results of immunity.

Hepatitis B

• Immunization with three (3) doses of vaccine required or titer results demonstrating immunity is required.
• Verification of the first two Hepatitis B immunizations is required by the required documents deadline.
• It is the responsibility of the student to provide verification of the 3rd Hep B as soon as it is required and completed which is usually 6 months from 1st and 2nd immunization.
• Date and copy of immunization proof or titer results are required.
• Proof of titer must include 1) healthcare provider signature, 2) date of signature, 3) license # or clinic stamp information and 4) results of immunity.

Varicella (Chickenpox / VZV)

• Immunization with two (2) doses of vaccine required or titer with results demonstrating immunity is required.
• Date and copy of immunization proof or titer results are required.
• Proof of titer must include 1) healthcare provider signature, 2) date of signature, 3) license # or clinic stamp information and 4) results of immunity.

Tuberculosis (TB / PPD/ Mantoux tuberculin (M) Screening Test

• Separate form will be supplied at orientation for TB testing.

Physical / health examination / Essential Functions:

• Form with list of "Essential Functions" will be provided at orientation.

CPR Certification

• Copy of CPR card front and back is required.
• CPR must be "BLS Healthcare Provider" from the American Heart Association

Health Insurance

• Copy of insurance card front and back is required and name of Policy Holder / Subscriber.
• Only UAH / United Health Care must provide active semesters for example: 2012-2013
• Student insurance may be obtained from United Health Care Insurance through Student Development Services 256-824-6203 / UAH University Center, Room 113 / www.studentinsurance.net .

Criminal Background Check and Drug Testing

• Form will be provided at orientation.

Health Examinations, Hepatitis B, MMR, and Varicella immunizations and TB testing are available for students at the UAH Student Health Center, Mon-Fri. 8:15 a.m. to 5 p.m. at 256-824-6775. Appointments are required.
