Charger 360 provides a centralized way for UAH community members to report concerns and incidents or address observed difficulties that may impede a person's success at UAH.

Academic Success Referral (Individual or Multiple Students)

Faculty who observe patterns of academic behavior in an individual student that may endanger their academic success are encouraged to complete this form. This includes concerns on attendance, academic performance/preparation, etc.

Attendance Referral (Multiple Students)

Refer multiple students for attendance concerns. This process serves as a means to connect with students that have two (2) consecutive absences or display a pattern of inconsistent attendance. Faculty can also report students who have never attended their course by completing this form. Note: Students who have never attended should be reported separately from students with attendance concerns.

Academic Performance Referral (Multiple Students)

Refer multiple students specifically for academic performance (i.e., multiple students struggling with academic content, consistent poor performance, etc.).

Student Conduct Referral

Report suspected violations of the Student Code of Conduct including reports of dishonesty, classroom disruption, disorderly conduct, etc.

Person of Concern Referral

Faculty, staff, and students may voluntarily report UAH community members who are experiencing crisis, displaying odd or unusual behaviors, or engaging in other behaviors that may be perceived as being harmful to themselves and/or others. When appropriate, this information will be automatically shared with the UAH Counseling Center.

Equal Opportunity/Title IX Complaint Form

Report alleged sexual harassment, sexual assault, dating violence, stalking, or other sexual violence; AND/OR discrimination, bullying, and harassment based on race, color, national origin, religion, sex (including pregnancy), sexual orientation, gender identity, age, disability, retaliation, and/or veteran status in violation of university anti-discrimination policies.

File a general complaint with the Office of the Dean of Students.

Report suspected Faculty/Staff violations of Health and Safety requirements to the Provosts Office and Human Resources.

Used by UAH Police Department staff only to report incidents of concern that require follow up including policy violations.

About Charger 360

The Charger 360 program is coordinated by the Office of the Dean of Students and uses Maxient software to manage the referral records. Through Maxient, we are able to centralize reporting and record keeping on disciplinary, academic success and attendance, person of concern, and discrimination/Title IX issues. Maxient helps us connect important information related to community members' conduct and well-being to provide coordinated support for success.
