Summary of Benefits Jan. 2008 UAH HIPAA Privacy Statement Blue Cross/Blue Shield HIPAA Privacy Notice Creditable Drug Coverage Notice-February 25, 2008 Eligibility: All full-time faculty and staff are eligible for enrollment in the heath insurance program. Coverage is required for all eligible employees. Enrollment must occur within the first 30 days of employment or within 30 days of a family status change (marriage, divorce, birth, death, or loss of coverage). Coverage may begin the effective date of employment or the first day of the following month. Waivers are granted for those who have comparable coverage through another group or individual policy by completing a health insurance waiver form. Open Enrollment: Health Insurance Open Enrollment is held in November of each year with changes to be effective January 1st. Individuals wishing to add or delete dependent coverage due to a change in family status (marriage, divorce, birth of children) may do so outside of open enrollment within 30 days of the status change. Premiums: Employee and UAH share the cost, with UAH contributing a fixed amount each month toward the single and family contracts. Employee contributions are made from pre-tax dollars meaning no state or federal taxes are paid on premiums.
