(regular full-time employees who work at least 32 hours or more per week)
Welcome to the University of Alabama in Huntsville!
New employee orientation is conducted every Wednesday morning at 9:00 a.m. in Shelbie King Hall Room 102 for full-time regular employees.
Step 1- Please print and complete the following forms:
Please read and follow the instructions below to begin your online orientation. The employee is responsible for making sure all paperwork is printed, filled out completely, and returned at your scheduled orienation with the Benefits and Employee Services Office. Failure to properly complete and submit all paperwork to Human Resources may result in delay of receiving your paycheck.
The following forms must be completed. Click below to download:
I-9 Form: This form must be completed within your first three (3) working days of employment. The I-9 form confirms identity and work eligibility. After you have read the instructions, please go to page 3 and complete Section 1 only. A University representative will complete Section 2 of the I-9 form. Two forms of identification are required. Some examples are U.S. Passport, drivers license, social security card, birth certificate, etc. (for an all inclusive list please refer to the back of the form I-9 which is mailed with offer letters from the Staff Employment Office).
Employee Profile: The employee profile allows the HR Office to collect personal information for the employee database. It provides biographical data, military history, educational data and emergency contact information.
Authorization for Direct Deposit: This form authorizes the UAH payroll office to deposit your payroll check electronically. You must attach a voided check or deposit slip as the bank routing number and account number are required.
Parking Decal: Vehicle Registration and Parking Decal are required for all faculty and staff employees that park on campus. This form authorizes payroll deduction for your vehicle(s) registration.
Alabama State Tax Form (A-4): State Employee Withholding Exemption Certificate.
Federal Tax Form (W-4): Federal Employee Withholding Allowance Certificate.
Drug Policy: The University is subject to the requirements of the Drug Free Workplace Act of 1988 and has adopted a policy for compliance with that Act.
Indebtedness Policy: This form confirms acknowledgement of the University accountability policy in which payroll deductions shall be used as a means of collecting delinquent debts owed by University employees to the institution, whether such debts are employment related or not and whether such debts are incurred during or prior to the period of employment.
