Voluntary Accidental Death & Dismemberment

Additional Accidental Death and Dismemberment is provided by MetLife.  

Enrollment:

Regular full-time employees may enroll at any time.

Coverage:

Employees may choose accidental benefits in varying amounts from $25,000 to $500,000. Level of coverage is dependent upon the amount chosen and if the accidental injury results in loss of life or total loss of sight, hearing, limbs, etc.

Employee Coverage: 100% of principle sum


If Family coverage is chosen your spouse and eligible dependent children are automatically insured as follows:

Spouse Coverage: 40% of principle sum; 50% if no eligible dependents


Dependent Children: 10% of principle sum; 15% if employee is a single parent; up to a $52,500 maximum (dependents must be unmarried dependent children from birth to 19 years of age, or to age 25 if attending college on a full-time basis)

Costs: Premiums are paid by the employee through monthly payroll deduction. The following table will help calculate the amount of premium due based on amount of coverage chosen:

Portability:Upon termination of employment, an employee may continue his or her voluntary AD&D coverage under the group policy. The employee has 30 days from date of termination to notify the company in writing.
